The camper online form, online medical packet and an universal health/immunization record (administered in the last 12 months) will be required for your child(ren) to attend camp. All forms are due by Friday, May 30th.

If you have more than one child, please complete and fill out the online form for each child. 

2. Medical Packet Older Division ONLY 

CLICK HERE for the online medical packet. This medical packet must be completed by a parent or guardian for all campers in the older division even if they attend LPS/TCA as we will be at the Alpine Scout Camp.

3. Upload Older Division ONLY

CLICK HERE to upload any forms that require a pediatrician's and/or allergist's signature, downloaded through the medical packet online form system including the medical and immunization record.
